CAID New Costume Reservation Form

I understand that Ms. Amanda will handle the entire costume process which includes:

(1) Reserving your costume spot

(2) Setting up an appointment and/or Google doc with you and your dancer to discuss your dancer's preferred design ideas and color palette

(3) Conveying that information to the costume maker

(4) Working with the costume maker on the design*

I understand that there is a $50 Costume Consultation Fee for this process which will be due before your spot has been reserved. *

I understand there is a deposit required to reserve a spot with the costume maker. I understand that this deposit is non-refundable & non-transferrable, NO exceptions. Should I decide to cancel my dancer's costume after submitting a deposit, I understand that I will not be refunded the deposit amount and I will forfeit my dancer's costume spot. I understand that if I decide to reserve a costume spot in the future, the costume process will begin again and another deposit will due at that time. *

I understand that I am reserving a costume spot within a general time frame this season. I understand that I will not receive a more specific delivery date until the design process is complete. *

I understand that the costume maker is NOT local, therefore my dancer is unable to be fitted in person. I understand this means that my dancer's costume may not fit perfectly and therefore, additional alterations may be needed when my dancer's costume arrives. I understand that due to the unique shape/style of Irish dance costumes, the Carpenter Academy has a preferred seamstress for costume alterations and that contact information for the seamstress will be provided upon request.*
