Studio Usage Waiver

Please fill out this form prior to studio usage. Only dancers that have completed this waiver are permitted in the studio without instructor supervison. If a group of dancers are using the space, ALL dancers must receive approval from Ms. Amanda AND complete this form prior to studio use.

I understand that by completing this form, I will not hold the Carpenter Academy of Irish Dance, Amanda Carpenter TCRG, Peter Gardner TCRG, Ashley McMahan TCRG, Oonagh Lynch Benson TCRG, Laura Kerr TCRG, Julia Nailor TCRG, Heather Gallagher, or any other instructors of the Carpenter Academy of Irish Dance responsible for any injury or illness obtained on the premises of:

I hereby release the Carpenter Academy of Irish Dance and/or its instructors of any and all legal liability.

I understand that by completing this form I am assuming the financial responsibilty for any property damage due to negligence/misuse of the studio space. Additionally, I understand that I am liable for the cost of utilities should I or my dancer fail to adhere to the following guidelines when exiting the studio:

Turning off all lights

Turning the A/C thermostat to "Off" / "Fan Auto"

Turning off and unplugging studio floor fans

Turning off speaker system

Closing curtains

Locking doors

I understand that a dancer under the age of 18 must have a parent/guardian stay on the studio premises in the studio parking lot at all times.

I understand that my dancer must inform Ms. Amanda via text message when arriving and departing from the studio. Additionally, to avoid a high utiliy fee, my dancer must send Ms. Amanda a photo of the A/C thermostat set to "Off" / "Fan Auto" upon leaving the studio.

Note - If a group of dancers are using the space together, the dancer with the studio key is responsible for sending the A/C thermostat photo.

I understand that the studio key must be returned to Ms. Amanda at the end of the designated time period. I understand that there is a "Lost Key Fee" of $100, which will go to cover the cost of new lock installation on the doors.
